Can my spouse call for information regarding my account?

Before any information can be released to your spouse, the participant must provide written authorization to HFS. This information should be faxed to HFS at 410.771.5533 or 888.510.4218 or mailed to HFS, P.O. Box 1550, Hunt Valley, Maryland 21030-1550.
